Admission Criteria
Those who meet the criteria of Department of Social and Health Services (DSHS) with Axis I diagnosis of major depression, bipolar disorder, schizophrenia, schizoaffective disorder, or Global Assessment of Functioning scale score of less than 50.

Adhering to the Resiliency and Disease Management statewide initiative, this program offers medication related services including prescription medication management, administration, training and support services as well as ongoing medication monitoring.

Admission Criteria
- Meet priority population as defined by Department of Social and Health Services (DSHS); and
- Eligibility determined by Uniform Assessment scores completed at intake or by the case manager.
A Brief Description
This program offers services provided by professional (Rehabilitation Specialist) and para-professional (Rehabilitation Technician) mental health staff. Services are provided to an individual to facilitate the maximum reduction of disability and restoration of the individual to their best possible level of functioning.
Services include: symptom management, acquiring the skills to cope with the demands of the environment, strengthening social support networks and tending to essential tasks needed to improve self-sufficiency. The services are tailored to the individual and address the individual’s specific strengths, needs, and self-stated life goals.
Services are provided in the community and in locations that best meet the individual’s needs. Services are provided in the form of individual and small groups. The effectiveness of these services is measured by the reduction of psychiatric symptoms and need for hospitalization, increased effectiveness in the community, and increased satisfaction with life.

Projects for Assistance in Transition from Homelessness (PATH)
Admission Criteria
- Age 18 or over
- Resident of Galveston or Brazoria County
- Homeless at point of contact
- Diagnosis of chronic, persistent major mental illness; also may have co-occurring severe, long-term substance abuse disorders
A Brief Description
PATH provides one-time assistance for security deposits, one-time rental payments, and housing start-up expenses. Program components include street outreach, screening, assessment, rehabilitative skills training, case management, referrals, and housing. Staff provide services that increase participants’ mental stability, improve their health, support their abstinence from alcohol and drug use, and help them locate safe and affordable housing. PATH provides crucial mental health and substance abuse interventions that support recovery. Program staff will meet with individuals on the streets, at the Gulf Coast Center’s Galveston office, or at the shelters where the individuals initially sought help. Following the first contact, PATH case managers will continue to meet with participants at the shelter, on the streets, or in arranged housing, developing a treatment plan with realistic goals and objectives.
PATH consumers who need longer term housing or more intensive rehabilitation may be referred to Transitional or Permanent Supportive Housing.

Transitional Housing Program
Admission Criteria
- Age 18 or over
- Resident of Galveston or Brazoria County
- History of chronic homelessness
- Diagnosis of chronic, persistent major mental illness of two or more years in duration; also may have co-occurring severe, long-term substance abuse disorders
A Brief Description
The Gulf Coast Center’s 12-bed Transitional Housing program serves homeless persons with mental illness or co-occurring disorders exclusively—and it is the only transitional housing provider in Galveston and Brazoria counties that does so. Transitional Housing clients enter the program from the Center’s PATH program or from other agencies’ staff (hospitals, jails, substance abuse treatment facilities, and shelters). An essential component after emergency shelter, Transitional Housing provides the second phase of housing for homeless individuals whose mental disabilities demand not only long-term treatment but also rehabilitation. Sheltered safely in two group homes in Galveston with 24-hour supervision, Transitional Housing clients receive the treatment and supportive services that they require to help them regain and sustain mental stability. Case managers and rehabilitation technicians either arrange or provide mental health and substance abuse monitoring, psychotropic medication monitoring, rehabilitative skills training, medical care, job training, and education. Also, through the Center’s clinics, clients enroll in all mainstream benefits programs for which they are eligible. Participation in Transitional Housing is limited to two years.

Permanent Supportive Housing
Admission Criteria

- Diagnosis of chronic, persistent major mental illness; also may have co-occurring severe, long-term substance abuse disorders
A Brief Description
Permanent Supportive Housing, the third step in housing services for persons with mental illness who are homeless, provides safe, affordable housing in individual apartments. Permanent Supportive Housing consumers may enter the program from the PATH program or Transitional Housing. Permanent Supportive Housing clients are those who can live without the degree of supervision that Transitional Housing consumers require. These individuals are stabilized on medication, not actively using alcohol or drugs, and are trained in daily living skills; some have acquired an income (usually from mainstream benefits programs). A Case Manager and Case Aides help participants locate safe, affordable, and suitable apartments in accessible complexes, provide rehabilitative skills training, accompany clients to appointments with physicians and psychiatrists, take clients grocery shopping, and monitor their apartments to ensure safe, sanitary living conditions. These staff also help participants access supportive services that build self-sufficiency: independent living skills; vocational and employment training; education; symptom management; socialization; and help acquiring mainstream benefits to increase their incomes. The Center has implemented three Permanent Supportive Housing programs in the service area, each of which serves 15 persons. Participation in this phase of housing is not time-limited.
