Make an appointment for the Applicant to come to the Gulf Coast Center for a Determination of Eligibility

Note: A packet with information on types of services will be mailed to the Applicant.

The Applicant will meet with a Gulf Coast Center psychologist.

STAR+PLUS PROGRAM EXPANSION

REMINDER - With the expansion of STAR+PLUS effective February 1, 2007, providers of long-term services and support contracting as a network provider for STAR+PLUS services should contact the HMO with questions and issues about the program.  If you are unsure of which HMO your provider contracts with, a call to your agency management staff may be of help.  It is not appropriate for providers to contact TMHP with billing questions and/or issues about an HMO.
